DESCRIPTION
Letrozole (Femara) was patented in 1986, approved for medical use in 1996, and is marketed under the brand name "Femara." It works by selectively blocking the aromatase enzyme, which converts androgens to estrogens in peripheral tissues. This effect sharply reduces circulating estrogen, inhibiting the growth of estrogen-dependent cancer cells in postmenopausal women. It is not effective in premenopausal women for breast cancer because their primary source of estrogen is ovarian, not peripheral, and is recommended as a first-line medication for ovulation induction in women with PCOS.
Letrozole (Femara) binds reversibly to the heme group of the aromatase enzyme cytochrome P450 unit, potently and specifically inhibiting peripheral estrogen synthesis. In postmenopausal women, it reduces estradiol, estrone, and estrone sulfate concentrations by up to 95%, without impairing adrenal steroidogenesis. Letrozole is considered more potent than other aromatase inhibitors like anastrozole and exemestane for this inhibition.
